患子宮頸癌難有下一代?醫生拆解4大症狀 1種療法準確率達95%可保生育能力

子宮頸癌佔香港女性常見十大癌症前列,成因與是HPV病毒有關,當中90%的子宮頸癌是因為持續感染高危形HPV。在十多種高危HPV中,7成的子宮頸癌由HPV16及HPV18 所引致。
子宮頸癌成因是甚麼?陰道分泌異常是警號?
當子宮頸細胞被高危形HPV持續感染,以致宮頸上皮組織出現不正常的細胞病變,這就是宮頸上皮內病變(簡稱CIN)。CIN分為「輕度」(CIN1)及「高度」(CIN2和CIN3)3個等級,如果HPV感染持續存在並導致CIN的進一步發展,最終可能會突破基底膜並形成宮頸癌。在這個階段,可能會出現諸如不規則出血、陰道分泌物異常等症狀。

患子宮頸癌難有下一代?傳統治療增早產風險?
其實,子宮頸癌是可以預防的癌症。接種疫苗、避免接觸香煙及電子煙、早期發現和積極治療可以有效提高治療成功率,避免從病變發展成為癌症的風險。若CIN1階段持續一年以上,約15% 可能會演變成「高度病變」(CIN2 /CIN3)。
「高度病變」的傳統治療方式是入侵性手術,主要是電環/激光/傳統錐形切割。在治療過程中極有可能導致出血、傷口發炎、宮頸收窄或變短,從而影響日後懷孕生育時增加早產等風險。

如何治療子宮頸癌?1種療法可保留生育能力?
隨著醫療科技的發展,希望生育的女性已經有了新的非侵入性治療選擇:「聚焦超聲治療」。此方法通過「由内向外」的無創治療模式,讓超聲波聚焦在表皮下達6mm的位置直擊病變組織、殺滅受HPV 感染的細胞、提高身體免疫力,而且定位準確、有效率達95%、復發率極低、宮頸表層組織保持完整。治療後,只會有一次過渡性的充血水腫,術後恢復更快、不良反應較小、無瘢痕,可保留生理功能和減少日後對懷孕過程的影響,更適合有生育需求的患者。
子宮頸癌有何常見症狀?
子宮頸癌初期可能沒有明顯的症狀,因此定期接受婦科檢查是非常重要的,以便早期發現和治療。常見的宮頸癌徵狀包括:
- 不尋常的陰道出血:月經週期外的出血、月經期間的異常出血、性交後的出血或更年期後的出血
- 異常陰道分泌物:包括異味、血絲、棕色或增加的分泌物
- 盆腔疼痛:可能出現盆腔疼痛或不適感
- 尿液或大便變化:如果宮頸癌擴散到附近的器官如膀胱或直腸,會出現血尿、排尿困難、腹痛或排便困難等症狀
