患子宫颈癌难有下一代?医生拆解4大症状 1种疗法准确率达95%可保生育能力

子宫颈癌占香港女性常见十大癌症前列,成因与是HPV病毒有关,当中90%的子宫颈癌是因为持续感染高危形HPV。在十多种高危HPV中,7成的子宫颈癌由HPV16及HPV18 所引致。
子宫颈癌成因是甚么?阴道分泌异常是警号?
当子宫颈细胞被高危形HPV持续感染,以致宫颈上皮组织出现不正常的细胞病变,这就是宫颈上皮内病变(简称CIN)。CIN分为「轻度」(CIN1)及「高度」(CIN2和CIN3)3个等级,如果HPV感染持续存在并导致CIN的进一步发展,最终可能会突破基底膜并形成宫颈癌。在这个阶段,可能会出现诸如不规则出血、阴道分泌物异常等症状。

患子宫颈癌难有下一代?传统治疗增早产风险?
其实,子宫颈癌是可以预防的癌症。接种疫苗、避免接触香烟及电子烟、早期发现和积极治疗可以有效提高治疗成功率,避免从病变发展成为癌症的风险。若CIN1阶段持续一年以上,约15% 可能会演变成「高度病变」(CIN2 /CIN3)。
「高度病变」的传统治疗方式是入侵性手术,主要是电环/激光/传统锥形切割。在治疗过程中极有可能导致出血、伤口发炎、宫颈收窄或变短,从而影响日后怀孕生育时增加早产等风险。

如何治疗子宫颈癌?1种疗法可保留生育能力?
随著医疗科技的发展,希望生育的女性已经有了新的非侵入性治疗选择:「聚焦超声治疗」。此方法通过「由内向外」的无创治疗模式,让超声波聚焦在表皮下达6mm的位置直击病变组织、杀灭受HPV 感染的细胞、提高身体免疫力,而且定位准确、有效率达95%、复发率极低、宫颈表层组织保持完整。治疗后,只会有一次过渡性的充血水肿,术后恢复更快、不良反应较小、无瘢痕,可保留生理功能和减少日后对怀孕过程的影响,更适合有生育需求的患者。
子宫颈癌有何常见症状?
子宫颈癌初期可能没有明显的症状,因此定期接受妇科检查是非常重要的,以便早期发现和治疗。常见的宫颈癌徵状包括:
- 不寻常的阴道出血:月经周期外的出血、月经期间的异常出血、性交后的出血或更年期后的出血
- 异常阴道分泌物:包括异味、血丝、棕色或增加的分泌物
- 盆腔疼痛:可能出现盆腔疼痛或不适感
- 尿液或大便变化:如果宫颈癌扩散到附近的器官如膀胱或直肠,会出现血尿、排尿困难、腹痛或排便困难等症状
